Guidelines for Eligibility

• 65 or older
• One to three critical needs, depending on level of program. (Bathing, Dressing, Eating / Feeding, Medications, Transferring, Toileting and Meal Preparation).
• Income eligibility – Call DSS 1(800) 445-5394
• Connecticut Community Care Inc. (CCCI) # (203) 596-4800 (Income varies depending on level needed).

Services Provided

• Adult day care: socialization out of home
• Nursing Home health aide: assist with bathing and personal care
• Homemaker: cleaning, laundry, shopping and meal assist Food service delivery
• Emergency Response System
• Companion: socialization in home
• Rehabilitative Services: PT, OT, Speech
• In Home Counseling
• Care Management of services: by CCCI Nurse or Social Worker
• Money Management: monthly bill paying / budgeting

All services are monitored and managed by a Care Manager employed by Connecticut Community Care Inc. and include monthly phone calls and visits every 6 months with client.
